Make a form available to a patient in PatientAccess

You may have patient types that require patients to complete a default form. For example, all Pre-Doc clinics at a dental institution may require their patients to complete a Medical History form. In axiUm, a Medical History form can be marked as a default form in the PreDoc clinics.

Important: Default forms must be marked as iForms. See Mark an EPR form as an iForm.

To make a default form available to a patient type:

1. In the Maintenance module, select the Patient tab.
2. Select the Default iForm Definitions icon to display the Default iForm Definitions window.
3. Click the Clear Data icon to clear the fields.
4. Enter the fields and options. For example, we will enter:

• Patient Type: Undergraduate Clinic
• Code: EPR, Medical History
• Access: Full

5. Click Add a new Record to add the item to the list view.

You have now assigned a default form to a patient type.
